Eligibility for Services

In order to receive most services at the Arc in Hawaii, participants need to be eligible for Medicaid Waiver. However, some services are also available on a fee-for-service basis for those able to pay for them. Eligibility requirements for Medicaid Waiver services include:

 

• Being found eligible for Medicaid through the Department of Human Services, MedQuest Division

• Being found eligible for Case Management Services through the Department of Health, Developmental Disabilities Division

• Being found eligible for Medicaid Waiver Services through the Department of Health, Developmental Disabilities Division

 

You are welcome to submit your Arc in Hawai'i Application For Services using the fillable application.  Please submit your application to the Arc in Hawai'i by email, fax or mail.

4.  Fill in the application using your computer

5. "Save as" Arc application for service

6. Attach to your email and submit to: info@thearcinhawaii.org
